Job Description
We are hiring an Adult Case Manager to join our Community Support Services Program. As a key player in transforming lives through recovery-focused, community-based care, you will help adults navigate mental health services, achieve goals outlined in their Individual Recovery Plans, and foster connections to natural supports and community resources.
Requirements
- Conducting community needs assessments and providing personalized case management
- Collaborating with an interdisciplinary team to support treatment plans
