Job Description
We are seeking a Highly Skilled, Caring, Compassionate Licensed Practical Nurse (LPN) to join our team as an MDS Coordinator. As an MDS Coordinator, you will perform nursing assessments of residents and documentation review and analysis, and lead or direct licensed and non-professional facility staff in the assessment and completion of MDS, CAA and Care Plan process.
Requirements
- Associates, Bachelor’s degree or university program certificate
- 2-4 years related experience and/or training or equivalent combination to education and experience required
- Experience as an MDS/Care Plan Coordinator preferred
- LPN or RN License
- Long term care experience preferred
