Job Description
Advanced Practice Provider (NP, PA, CNS) to provide primary and/or specialty care in a variety of healthcare settings, emphasizing health promotion, disease prevention, and diagnosis and treatment of acute and chronic health problems.
Requirements
- Nationally certified Advanced Practice Provider
- Conducts thorough medical histories and physical examinations
- Initiates lab, radiology tests, and special tests required for evaluation of illness
- Works in collaboration with the healthcare team
- Performs routine and specialized procedures
- Prescribes and monitors medications
- Educates patients on their disease, treatments, and related drug and treatment side effects
- Participates in medical team rounds and collaborates with nursing and other healthcare team members
- Refers patients to specialists as necessary
- Ensures continuity of care and participates in quality monitoring
- Facilitates discharge planning and regulatory standards compliance
