RN Case Manager

Join Cleveland Clinic's Main Campus as an RN Case Manager and work with a passionate team to facilitate transitional care coordination and empower patients and families to self-manage. Work alongside inter-professional teams to promote synchronization throughout the acute episode and ensure compliance with standards of accrediting bodies. As an RN Case Manager, you will float between various departments and locations at Cleveland Clinic Main Campus.

Requirements

• Graduate from an accredited school of Professional Nursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) Or the American Red Cross
• Three years of recent RN clinical experience
