Job Description
The Patient Navigator role is a community health professional that focuses on guiding patients through the abortion care system, overcoming obstacles, and improving access to care. The role involves scheduling appointments, case management, communication with patients and healthcare providers, and navigating financial and travel arrangements.
Requirements
- Organizing schedules and managing appointments for patients
- Providing effective case-management to a varying caseload of patients
- Facilitating communication between patients, support persons, and healthcare providers
- Monitoring multiple mediums of communication for referral intake
- Collecting and organizing patient data, drafting correspondence, and making financial and/or travel commitments
