CNS Healthcare's Case Manager is responsible for providing community-based case management services, primary and behavioral health integrated services. They will respond to the needs, desires, and interests of individuals, provide weekly community-based interventions, and facilitate weekly groups. They will also complete required documentation, develop and implement Person-Centered Plans, and match prevention, intervention, and resolution techniques to the particular circumstances of each individual.
Requirements
- Qualified Mental Health Professional (QMHP)
- Bachelor's Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field
- Specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has a mental illness
- Valid and unrestricted Michigan Driver's License
- Reliable transportation and current proof of auto insurance
- Working knowledge and understanding of mental illness, medications used in treating psychiatric disorders, and experience with other interventions used to treat psychiatric disorders.
