The Anchor Nurse Practitioner provides direct patient care, supports timely completion of care, and co-manages selected patients with primary care providers. The role requires a flexible and motivated team player with a minimum of 2 years of experience working as an NP.
Requirements
- Flexible, motivated team player who is committed to community health and the Patient-Centered Medical Home model.
- Minimum of 2 years of experience working as an NP
- Medical fluency in Haitian Kreyol, Spanish, French, Portuguese, or Cape Verdean Creole desired.
- Successful completion of an accredited Nurse Practitioner program or graduate of a Master’s Degree program with an incorporated Nurse Practitioner Program.
- Licensure: Licensed to practice in the Commonwealth of Massachusetts as a Nurse Practitioner.
- Active DEA & Massachusetts Controlled Substance licensure.
- Certifications: Active Certification from a recognized national certification program.
- Active Basic Life Support Certification.
