Hospice Medical Social Worker- PRN

As a Hospice Medical Social Worker (MSW), you will provide compassionate care to patients facing end-of-life issues. Your role involves emotional support, medical coordination, psychosocial evaluation, bereavement services, and community education outreach. This position is considered a "Safety-sensitive" role, subject to disciplinary action for marijuana use.

Requirements

• Participates in referral evaluation and documentation.
• Assesses patients and their families for psychological needs and concerns.
• Coordinates psychological care based on policies.
• Provides physical, emotional, and spiritual support.
• Documents all visits and calls, and places documentation in clinical records.
• Coordinates or participates in team meetings.
• Utilizes all available resources to assist patients and families.
• Serves as a member of IDT and attends team meetings.
• Responds to patient/family needs promptly.
