Home Health Hospice Registered Nurse
2-5 years of RN experience, BSN/MSN degree, and Oklahoma RN licensure required
Hospice Nurse Practitioner-PRN
Master's degree in nursing, 1 year Hospice experience preferred, Current STATE RN and Advanced Practice Nurse License
Hospice Medical Social Worker- PRN
MSW degree, 1-2 years hospice/HCPF experience. Valid Driver's License, Current Auto Insurance & AHA BLS, and current MSW/LCSW are required. Proficiency with Microsoft Office suite & strong communication skills needed
Home Health LPN Field Nurse
High School Diploma or Equivalent, Licensed Practical Nurse License for the State of Oklahoma, ACLS, CPR
Dialysis Nurse
RN license in OK, BLS/ACLS/PALS certification, and current auto insurance. Knowledge of medicare and applicable state standards also needed
Home Health/Hospice Aide (PRN)
High school diploma or GED. Home health/hospice experience preferred. Valid driver's license and CPR/CNA/HHA certifications required.
Home Health Registered Nurse CM
Must be a licensed Registered Nurse in Oklahoma. Home Care/Case Management experience preferred. Excellent communication/time-management skills are needed
Home Health/Hospice Aide (PRN)
High school diploma/GED and current CDA certification. 1+ year experience preferred. Valid driver’s license, insurance and CPR certification required
Hospice Nurse Practitioner-PRN
Master’s in Nursing, RN license, and active Driver's License are required. Strong patient care and collaboration skills are vital
Chaplain- PRN
Accredited seminary/theology or equivalent training. 1+ yrs hospice exp. CPR/BLS certification. VA exemption req. Computer proficiency
