Job Description
Commonshare is seeking a compassionate Advanced Nurse Practitioner to provide comprehensive primary care services to elderly residents at Imperial Plaza, a senior living community in Richmond, Virginia. The role focuses on personalized care, managing chronic conditions, and reducing hospitalizations. The position is part-time with a flexible schedule.
Requirements
- Current license as a Nurse Practitioner (NP) in the state of Virginia
- Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner (AGPCNP or FNP) strongly preferred
- Minimum of 2 years' experience in geriatric, internal medicine, or primary care settings
- Strong clinical assessment skills and comfort with managing multiple chronic conditions in older adults
- Excellent communication and interpersonal skills with a patient-centered approach
- Experience working in residential or long-term care settings is a plus
