
Job description
The Diabetes Educator will work with a multidisciplinary team to develop and teach diabetes education programs to patients with Type 1 and Type 2 diabetes. The role requires collaboration with a team to provide quality care and act as a leader in improving patient outcomes.
Teach diabetes education care skills to patients, develop and maintain program development, policies, marketing, and data outcomes. Collaborate with a multidisciplinary team to provide quality care for patients enrolled in the diabetes program.
The ideal candidate will have experience in patient education, specifically in diabetic education, and be a Certified Diabetic Educator. A Registered Dietitian or Registered Nurse in the state of employment is also required.
