Job Description
Conduct home visits to assess family needs, identify community supports, and develop a Family Support Plan. Support families in accessing services, benefits, and DDS supports. Practice trauma-informed care and assist with obtaining entitlement benefits, clinical and medical supports, and flexible funds.
Requirements
- Master's degree in a human services-related field
- 2+ years of experience providing direct support to families
- Valid Driver's License and acceptable driving record
- Pass company background and reference checks
