Care Manager

Requirements

• Bachelor’s Degree in a Human Services or closely related area
• Minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities
• Eligibility for long-term care functional screener certification
• Valid driver's license and proof of required insurance coverage
