Job Description
As a member of the interdisciplinary team, the RN Case Manager provides skilled nursing assessment, care planning, and interventions to maximize the comfort and quality of life of hospice patients and families.
Requirements
- Graduation from an accredited school of nursing
- At least one-year recent experience within the last three years as a professional nurse or BSN from a program accredited by the National League of Nursing
- Possession of a current license to practice as a registered nurse in the State of California
- Possession of a valid California driver’s license and current automobile insurance
- CHPN preferred
- Possesses and maintains current CPR certification
