Social Worker MSW

Social Worker (SW) responsible for making psychosocial assessments of terminally ill patients and their families, and providing supportive counseling for dealing with anticipatory grief and bereavement issues.

Requirements

• Completes initial psychosocial assessment on all patients/families, as assigned
• Evaluates coping skills in crisis situations and anticipatory grief issues
• Documents psychosocial assessment, treatment plan and all visits and significant phone contacts
