Residential Case Manager - Community Bridge Program

Residential Case Manager provides case management and supportive services to residents in CHS Residential programs, working with other staff members to complete required documentation and manage crisis situations. The role involves developing professional relationships with residents, providing direct case management, and facilitating opportunities for residents to connect with local community resources.

Requirements

• Develop a professional and therapeutic relationship with residents.
• Maintain and model appropriate personal boundaries.
• Provide direct case management to residents regarding their goals, personal budget, personal needs, and interests.
• Follow communication protocol and address complaints, behavioral problems, etc. promptly.
• Collaborate with Housing and Employment Coordinators to develop individualized goal plans with residents.
• Assist with planning and coordinating participant enrollment in life skills workshops and training sessions.
• Facilitate opportunities for residents to connect with local community resources, volunteer opportunities, educational programs, and employment services.
• Complete required daily written documentation in a timely manner.
• Provide individualized coaching and support to help residents develop essential life skills and increase self-sufficiency.
• Complete data entry of information into the agency database and any funder-specific site for reporting purposes on a timely basis.
• Maintain the physical site by prompting residents to perform household duties and report any needed facility-related repairs.
• Attend necessary training and professional development sessions to enhance agency-wide knowledge.
• Complete all job duties in accordance with the mission and values of CHS.
