Community Integration Counselor

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities. We are seeking a Community Integration Counselor to assist individuals with learning physical, intellectual, emotional, and social life skills.

Requirements

• Support and encourage individuals to increase their ability to be as independent as possible
• Meet with waiver participants on a regular basis
• Collaborate with the case manager to develop and facilitate programs
• Provide ongoing support for community integration efforts
• Document individual progress
• Assist in building a support network between the individual and the community
• Ensure medical consultation forms are completed
• Administer medications and ensure proper completion of necessary documentation
• Coordinate all leisure and volunteer opportunities
• Monitor the health and medical needs of individuals
• Drive individuals to medical appointments and activities
• Ensure program documentation and billable records are completed accurately and timely
