We are seeking a Registered Nurse in Hillsborough, NJ to deliver care to individuals with disabilities in a community setting.

Requirements

Provide treatment and health care procedures ordered by a physician
Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
Advise supervisor of all concerns and issues related to the provision of nursing services
Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team
Train employees on the medical needs of assigned individuals
Monitor health and medical conditions of individuals and refer for medical services as needed
Monitor the use of medications through medication administration logs
Ensure health service supplies are available and replenished as needed
Ensure program documentation and billable records are completed accurately and timely
Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
Participate with individuals in community, social, and recreational activities
Participate in circles of support for personal futures planning
Follow strict OSHA procedures regarding blood borne pathogens
