Case Manager

Requirements

• Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families
• Supports and closely coordinates mental health care with the consumer’s clinical care team and the primary care provider
• Attends collaborative treatment team meetings and reviews of consumers
• Provides crisis de-escalation when needed
• Assists eligible consumers in gaining access to needed medical, social, educational, and other services
• Provides an assessment of needs and service plan development
• Empowers consumers to reach their goals and sets a positive example and provides role modeling as well as technical expertise to accomplish goals
• Provides transportation for consumers in agency vehicles when needed
• Works closely with outside organizations on resources and care coordination
• Uses an evidence-based model of measurement-based care to track success for each individual consumer
