Nurse, RN

Community Reach Center is looking for a compassionate RN to join their team in providing exceptional mental health outcomes. The RN will work in a residential, outpatient, or outreach environment, providing care to consumers in a therapeutic setting. The role requires a valid Colorado RN license, associate or bachelor's degree in nursing, and a valid driver's license. Requirements

• Assess needs of client, develops solutions, and ensures implementation of solutions/interventions and programs to meet those identified needs.
• Provides case management and other clinical interventions as appropriate.
• Aids in crisis intervention, as needed, and notifies appropriate team(s), Manager(s) or 911.
• Provides client and family education either in groups or individually.
• Participates in care coordination, collaborative development, and implementation of client’s treatment plan with all members of the treatment team.
