Job Description
Support individuals in attaining the highest quality of life while maintaining their safety across all settings.
Requirements
- Implement Positive Behavior Supports (PBS)
- Implement and follow individuals’ Behavior Skills Plans
- Implement and follow EO509 Dining Guidelines
- Participate in and complete Licensing and Certification paperwork
- Follow and Implement individuals’ daily schedules
- Enhance an individual’s development by providing job coaching
- Maintain appropriate and professional boundaries with the individuals
- Involving individuals in social and vocational opportunities
- Implementing Individual Service Plan (ISP) objectives
- Promote a high quality of living for the individuals
- Support and accompany individuals in the community
- Record accurate and quality data on each individual throughout the shift
- Provide supervision of the individuals and ensures their safety in the residence and community
- Promote a high quality of living that includes routine maintenance/housekeeping
- Provide Transportation for individuals
- Perform verbal or physical interventions as needed to ensure safety
- Conduct physical restraints as needed
- Maintain HIPAA, Confidentiality and Cyber Security
