Family Service Care Coordinator provides case management and care coordination services to children, adolescents and families.
Requirements
- BSW/BA or AA and/or related experience
- Demonstrated case management skills and experience
- Knowledge of child development, emergency intervention methods, family dynamics, health and mental healthcare systems
- Case management certificate or other certification required by program or willingness to obtain certification
- Valid Driver's License and proof of liability insurance
- Reliable transportation
- Excellent oral and writing skills
- Bilingual in Spanish, preferred
