Care Coordination Supervisor

The Care Coordinator Supervisor is responsible for supervising care coordinators and navigators, providing medical, behavioral, psycho/social care, and coordinating long-term services and supports to members.

Requirements

• Licensed Registered Nurse (RN) or Licensed Nurse Practitioner
• 3-5 years of experience in a healthcare, patient oriented customer service position
• Experience with the populations similar to the Medicare Enrollee population, including community based and facility-based people with medical, LTSS and/or behavioral needs
• Case management experience preferred
• Disease management experience preferred
