Job Description
The RN Care Manager will be responsible for daily nurse visit schedule, triage, care management, and case management for patients. They will also analyze preventative screening data and provide feedback to nurse manager.
Requirements
- Graduate of an accredited RN nursing program
- Current New York State license in Registered Nursing
- Completes additional training for care management, SBIRT, Depression and Anxiety screening
- Demonstrate a willingness to accept direction from providers and Sr. VP/CNQO
- Possess the ability to work well and with compassion in stressful situations
- Maintain a professional and courteous demeanor with both patients and co-workers
- Be keenly aware of the importance of confidentiality in all aspects of the position
