Job Description
We are seeking a Case Manager to provide home-based case management and supportive mental health services for formerly homeless families housed throughout Contra Costa County. The ideal candidate will have a Master's degree or substitution of experience, two years' experience in the human services field, and be familiar with the needs and issues of homeless families with mental disabilities.
Requirements
- Master's degree or substitution of experience
- Two years' experience in the human services field
- Familiarity with the needs and issues of homeless families with mental disabilities
- Bi-lingual in English/Spanish (a plus)
- Outstanding written and verbal communication skills
- Computer proficiency in Microsoft and database applications
