Community Health Worker

The Community Health Worker (CHW) supports patients and families in accessing community services and resources. The CHW promotes health behaviors and works with the care team to create individualized care plans. They provide social support, advocacy, and education to patients and families.

Requirements

• Valid California Driver's License
• High school graduate or equivalent required
• Successful completion of a Community Health Worker formal training program
• Written and oral fluency in English and Spanish is preferred
• Experience working in a multi-cultural setting
