Job Description
COPE Health Solutions is seeking a Community Health Worker to navigate and access community services, foster healthy behaviors, and provide support to patients and families. The CHW will provide social support, advocacy, and educational resources, maintaining trust with both patients and families. This role focuses on improving self-care and health management and acting as a liaison between patients and services.
Requirements
- Valid California Driver’s License
- High school graduate or equivalent; Associate's Degree in Business Administration or related field preferred
- Successful completion of a Community Health Worker formal training program
- Willingness to learn and understand a variety of different cultures, perspectives, and norms.
- Basic computer skills required; electronic medical record (EMR) experience preferred
