Job Description
COPE Health Solutions is seeking a Community Health Worker to provide field-based member outreach services to Medicare Advantage (MA) plan members. The role involves building relationships with members and providers to identify barriers to care and promote member health. Work requires daily contact, scheduling, and tracking of outreach progress.
Requirements
- Valid NY Driver’s License
- High school graduate or equivalent or Associate's Degree in Business Administration preferred
- Fluent in both English and Spanish (preferred)
- Experience working in a multi-cultural setting (preferred)
- Experience in a community-based setting (preferred)
- Good communication and interpersonal skills
- Willingness to learn new things
- Strong customer service skills
- Detail-oriented
