Job Description
This LCSW/ECM role focuses on providing care management services to patients needing navigation and addressing patient care needs within a primary/telephonic setting. The position involves assessing patient risk, providing behavioral health support, and coordinating referrals with various providers and community resources. It requires collaboration with a multidisciplinary team to ensure patient well-being and optimize care.
Requirements
- Valid and current MSW, LCSW or LMSW licensure
- 3-5 years’ care management and/or managed care experience
- Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare, ambulatory care management, or managed health plan settings
