Job Description
This position is responsible for delivering comprehensive medical, preventive, and therapeutic health care services through a clinic-centered approach, including home visits and community outreach.
Requirements
- Assess health status and practices of individuals and families through histories, observation, and physical exams.
- Develop and implement treatment plans with periodic follow-up.
- Prescribe medications and treatments for common health conditions.
- Order, perform, and interpret laboratory and diagnostic tests.
- Coordinate patient follow-up with Community Health Aides and RN Case Manager.
- Maintain accurate and compliant medical records in the EHR system.
- Establish collaborative relationships with IHS, tribal, and non-tribal providers.
- Refer patients to specialty clinics and external providers as needed.
- Guide and support the organizationās vaccination and Community Health Aide programs.
- Assist in formulary development with medical, pharmacy, dental, and behavioral health staff.
- Participate in community wellness initiatives.
- Plan, evaluate, and coordinate clinic services and procedures.
- Perform other duties as assigned.
