Housing case management support to disabled individuals in a supported housing program, including education, service planning, and housing coordination.

Requirements

• Currently Licensed social worker (RSW, CSW, LMSW, LCSW) from a program accredited by the Council on Social Work Education
• BA/BS degree
• 3 years of experience providing social services
• Reliable transportation and a valid driver's license
• Valid car insurance
• No more than one at-fault accident in the past three years and no more than two moving violations in the past five years
