Job Description
The Service Coordinator (Mental Health Case Manager) is responsible for assessing clients, identifying problem areas, and coordinating treatment plans. The role involves working with the interdisciplinary team to ensure continuity of treatment efforts and implementing treatment plans.
Requirements
- Two (2) years of full-time experience or part-time equivalent in a mental health setting.
- Bachelor's degree with a major in psychology, social work, or behavioral sciences, or equivalent experience.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.
