Job Description
Signify Health, part of CVS Health, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The clinician will primarily travel up to 100% of the time, conducting virtual visits as needed and providing all Signify Health services, including Diagnostic & Preventive Services products.
Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within Washington and flex to other areas of business need
