We are seeking a Manager, Patient Support & Insurance who will engage renal discharge planners and case managers to build and strengthen relationships, ensuring a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients.
Requirements
- Consistent outreach to discharge planners to provide education and build awareness of offerings
- Partner with internal field teammates to optimize the admission process and implement best demonstrated practices
- Assist with necessary communication between hospital, admission and clinic teammates to work through patient placements with extenuating circumstances
- Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers
- Develop comprehensive knowledge of local market
