Registered Nurse RN Case Manager Experienced (IKC-CA)

We're seeking an experienced Registered Nurse RN Case Manager to join our DaVita IKC team. This hybrid, field-based role requires a dedicated home office and travel to an assigned territory within Southeast LA County. The successful candidate will coordinate and facilitate quality, cost-effective care for ESRD and CKD patients, working collaboratively with the healthcare team to minimize fragmentation of the healthcare delivery system.

Requirements

• A.D.N degree from accredited school of nursing required
• Current Registered Nurse (RN) license in the state of practice required
• Minimum of five (5) years’ experience in clinical nursing is strongly preferred
• Minimum of three (3) years’ experience in renal nursing preferred
• Case management experience is required
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
