Job Description
The Care Manager will provide case management assessment, person centered planning and documentation, referral and linkage, and monitoring/follow-up for clients. The role will also involve crisis intervention consultation, coordination, and outcomes management.
Requirements
- Bachelor's degree in a human service field
- Two years of MH/SA/DD experience with the population served
- Licensed RN with two years MH/SA/DD experience with the population served
- TCM trainings required to complete as assigned
