Care Manager Extender

Care Manager Extender provides care management functions, documentation, referral and linkage, and monitoring/follow-up. Under direct and indirect supervision, the Care Manager Extender refers and links to needed services, monitors/follow up with client and referrals, provides education for health promotion, and participates in interdisciplinary treatment planning.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists, Community health workers, or Individuals who served as Community Navigators
• 2 years of paid care management type experience with at least 1 year paid experience at any time with population served
• TCM trainings will be required to complete as assigned
