The Behavioral Health Social Work Case Manager plays a vital role as a core member of the BH team, coordinating mental and physical health care needs of patients on an assigned caseload and from internal PCP referrals.
Requirements
- Licensed Clinical Social worker, Licensed Professional counselor or Licensed in a related field of study
- Completion of degree and all post-degree supervision for licensure and at least 2 years of post-degree experience
- Experience of successfully delivering evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders
- Ability to effectively engage patients in a relatively short-term therapeutic relationship, via video and/or audio
- Evidence of high degree of organizational, collaborative and communication skills
