Care Coordinator I

As a Care Coordinator I, you will provide comprehensive case management, mental health, and substance use services to individuals with a history of persistent mental illness and/or substance use. You will offer personalized care and support to individuals within an integrated health care setting, helping them navigate and access necessary services to optimize their well-being and recovery.

Requirements

• A minimum of two (2) years of related experience or a bachelor’s degree is required
• Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients
• Current California driver’s license, car insurance, and a driving record acceptable to the Agency’s insurance carrier
