Care Coordinator I, Bilingual (Armenian)

This is a hybrid position based in Glendale, CA. You have the option to elect into a 9/80 flex work schedule. The Care Coordinator I, Bilingual - Armenian provides comprehensive case management, mental health, and substance use services to individuals with a history of persistent mental illness and/or substance use. In this position, you will be responsible for offering personalized care and support to individuals within an integrated health care setting, helping them navigate and access necessary services to optimize their well-being and recovery.

Requirements

• Fluency in required language for assigned program (Armenian).
• A minimum of two (2) years of related experience or a bachelor’s degree is required.
• Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients.
• Current California driver’s license, car insurance, and a driving record acceptable to the Agency’s insurance carrier.
