DOCS Medical Group seeks a Primary Care Provider to deliver and manage direct patient care for a diverse patient population. The ideal candidate will possess a valid Medical License, work authorization, and a Doctorate degree (Preferred) and Board Certification or eligibility. Proficiency in Electronic Health Records (EHR) systems is also required.
Requirements
- Conducting physical examinations, evaluations, assessments, diagnoses, and treatment plans for patients
- Providing value-based care by engaging patients and their families or caregivers in understanding, setting, and monitoring self-management care plans
- Documenting individualized care plans and coordinating care in the patient’s medical record
- Analyzing patients' medical history, medication allergies, physical condition, and examination findings to determine the most appropriate care
- Prescribing medications and treatment regimens as appropriate to assessed medical conditions
- Referring patients to specialists or internal medicine when necessary
- Directing and coordinating patient care activities with nursing and support staff as needed
- Upholding and promoting patient rights and privacy
- Responding appropriately to emergency situations
- Participating in meetings to evaluate practice processes, workflows, patient care, and quality improvement efforts
- Engaging in specified health promotion, education, and prevention programs
- Participating in population health activities as directed by the practice
- Adhering to departmental policies, procedures, objectives, quality improvement initiatives, and safety, environmental, and infection control standards
