Job Description
NeighborHealth is seeking a Care Navigator to work with patients' primary care teams to address non-medical needs that can affect health and well-being. The Care Navigator promotes patient-centered, comprehensive, and coordinated care, connecting patients with community resources and supporting them in navigating medical, social service, and legal systems.
Requirements
- Bachelor Degree
- 1-2 years in a direct service role
- Strong second language skills (Spanish preferred)
- Ability to multitask with strong organization and time management skills
