Job Description
Eastern Health is seeking a Team Leader to lead the Chandler House Continuing Care Team, providing case management for adults with severe mental illness. The role involves managing day-to-day operations, leading incident response, and promoting sustainable, continuous improvement.
Requirements
- Over ten years working in public mental health in a variety of settings
- Highly developed interpersonal and leadership experience
- Demonstrated change leadership experience
- Current registration with AHPRA, or eligible for AASW membership
- Strong commitment to high quality care
