Job Description
Integrated Case Management (ICMS) Case Manager will coordinate and manage the care of consumers in ICMS, providing ongoing support to help them function independently and access mental health services.
Requirements
- Valid and active driver's license
- Master's degree in social work, counseling, psychology or related field, or Bachelor of Arts in social work, counseling, psychology or related field with relevant supervisory experience
- One year of relevant supervisory experience in mental health
