Care Coordinator

We are seeking a Care Coordinator to work with children with developmental delays, helping them achieve their goals and thrive. As a trusted source of support, you will work alongside families to create a rich learning environment and provide individualized treatment.

Requirements

• Bachelor's degree in Social Work, Social Service, Education or equivalent
• 1 year minimum experience in professional social work
• Experience working with children with special needs in early intervention
• Significant knowledge of developmental disabilities, ADA, and IDEA guidelines
• Ability to work with families, professionals, and paraprofessionals in a Primary Service Provider model
• Clear and concise verbal and written communication skills
