Field Advanced Practice Provider

Field Advanced Practice Provider provides clinical and non-clinical support to patients in palliative care within a designated territory. Collaborates with company physicians, patients' other physicians, and providers to develop complex plans of care. Travels to client sites or designated locations as required.

Requirements

• MS in Nursing
• Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s)
• DEA registration or eligibility
• CPR certification
• 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care
• Bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) experience
• Experience working with Electronic Medical Records (EMR)
• Bilingual in English/Spanish
