Field Advanced Practice Provider

Field Advanced Practice Provider is a field-based role that enables associates to operate in the field, traveling to client sites or designated locations, with occasional office attendance for meetings or training. The position supports patients in palliative care within a designated territory, developing and implementing clinical plans of care and educating patients and families about medication usage and illness progression.

Requirements

• MS in Nursing
• Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license
• CPR certification
• 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care
