Convergence Health is seeking a strong clinician and motivated Pre-Assessment Testing (PAT) RN to provide safe, efficient, and effective patient focused care across multi-specialty for patients from infancy to geriatrics. The ideal candidate will cultivate strong relationships with Providers, and patients, while delivering exceptional, quality, patient-centric care as well as being an integral part of our team.
Requirements
- Patient Assessment: Conduct a comprehensive pre-operative assessment via phone to review medical histories as well as severity of comorbidities to identify potential risks and contraindications for anesthesia and surgery.
- Medication Management: Thorough review of all medications, including weight loss aids
- Testing and Diagnostics: Review necessary preoperative diagnostic tests, such as lab work and EKGs, ensure they are ordered by the provider and returned for review before scheduled procedure when applicable.
- Patient Education: Educate patients and their families on surgical procedures, anesthesia, and post-operative care.
- Care Plan Development: Collaborate with anesthesiologists, surgeons, and other staff to develop individualized care plans and coordinate patient care.
- Risk Management: Perform safety risk assessments for issues like falls, pressure ulcers, and skin breakdown.
- Documentation: Maintain accurate and complete patient records and documentation.
- Collaboration: Work closely with the surgical and anesthesia teams and other staff to ensure a comprehensive approach to pre-operative care.
