Care Coordinator, Embedded (Savannah, GA)

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Savannah, Georgia area.

Requirements

• Legally authorized to work in the United States of America
• Active, unrestricted clinical certification or licensure
• Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
• Proficiency with Microsoft 365, including Outlook, Word, and Excel
