Physical Therapist (PT) for Home Health

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier excellence of care to adults of all ages in the San Bernardino area. The Physical Therapist will assess and evaluate patient care needs, determine treatment plans, and monitor progress. The role requires remarkable knowledge of physical therapy principles, great communication skills, and the ability to maintain accurate records.

Requirements

• Physical Therapist license and registration by the state
• Completion of a Physical Therapist curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association
• Ability and enthusiasm in working with all ages
• Bilingual a plus!
